Short Class Descriptions

Care Coordination – Basic Training: The basics of providing the service of Care Coordination to a HCB Waiver Participant. Topics include: Service principles; HCB waiver processes; Eligibility; Cost-of-care co-pay; Creating a person centered plan of care; Certification and enrollment; Reporting; and Interfacing with SDS supports. Class REQUIRED to apply for certification as a Care Coordinator.

Training is offered either:


Care Coordination – Advanced: For Care Coordinators currently certified for 24 months or more only. Topics include but are not limited to: Care Coordination from the individual and family perspective; Eligibility- beyond the basics; Person Centeredness; Community resources: Regulations review; Documentation; and Providing Care Coordination in partnership with SDS.

Training is offered either:

 

PCA Orientation:The basic requirements to apply for certification as a PCA service provider agency. Topics include: Service principles; Service definitions; Definitions of Agency Based PCA and Consumer Directed PCA; Responsibilities as a provider; and Interfacing with SDS supports. REQUIRED to apply for certification as a provider agency.

Training is offered either:

 

Residential Supported Living in an Assisted Living Home:The basic requirements for providing residential service in an Assisted Living Home. Topics include: Service principles; Delegation in the ALH; Person centered services; Understanding a resident’s circle of care; Responsibilities as a provider; Reporting; and the Long Term Care Ombudsman’s office.

Training is offered:

  • In a classroom once a quarter
  • In a 3 week webinar offered occasionally

 

Critical Incident Reporting and Critical Incident Improvement Plans: Provider agencies choose staff to attend the webinar, and then train additional staff at their own agency. All CIR/CIIP learning materials are available to participants after attending the webinar, to use for this purpose.
