State Performance Measures
- Percentage of mothers of newborns who say their physician or health plan would not start prenatal care as early as they wanted or they could not get an appointment as early as they wanted.
- Percent of women who smoked during the last 3 months of pregnancy among women who smoked 3 months prior to pregnancy and were talked to about the effects of smoking by a prenatal health care provider.
- Percentage of children ages 10-11 who are at-risk for being overweight.
- Rate of substantiated reports of harm to children per one thousand children age 0 to 18.
- Percentage of women who recently had a live-born infant who reported their prenatal health care provider advised them not to drink alcohol during their pregnancy.
- Prevalence of unintended pregnancies that resulted in a live birth among women who reported having a controlling partner during the 12 months prior to getting pregnant.
- Percentage of women who recently had a live-born infant who reported that they always or often felt down, depressed, or hopeless since their new baby was born.
- Prevalence at birth of Fetal Alcohol Spectrum Disorders (FASD).
- Percentage of infants who are reported to have a Cleft Lip/Palate defect who access the Title V sponsored Cleft Lip and Palate Specialty Clinic within the first year of life.
National Performance Measures
- The percent of screen positive newborns who received timely follow up to definitive diagnosis and clinical management for condition(s) mandated by their State-sponsored newborn screening programs.
- The percent of children with special health care needs age 0 to 18 whose families partner in a decision-making at all levels and are satisfied with the services they receive.
- The percent of children with special health care needs age 0 to 18 who receive coordinated, ongoing, comprehensive care in a medical home.
- The percent of children with special health care needs age 0 to 18 whose families have adequate private and/or public insurance to pay for the services they need.
- The percent of children with special health care needs age 0 to 18 whose families report the community-based service systems are organized so they can use them easily.
- The percent of youth with special health care needs who received the services necessary to make transitions to all aspects of adult life.
- Percent of children 19 to 35 months who have received a full schedule of age appropriate immunizations for Measles, Mumps, Rubella, Polio, Diphtheria, Tetanus, Pertussis, Haemophilus Influenza and Hepatitis B.

- The percent of women (15–44) with a live birth during the reporting year whose observed to expected prenatal visits are greater than or equal to 80% on the Kotelchuck Index.
