3.	How much money other than earnings did your household receive last month?	$ .00	Include unemployment, retirement, child support, Social Security, etc. from prior month. Do not include PFDs or senior benifits. How do I report income?
4.	Do you live in subsidized rental housing?	Yes No	Rental housing subsidized by a government, housing authority, or non-profit entity, in which tenants pay 30 percent or less of their adjusted monthly income for rent.
5.	Do you live in housing that includes the cost of heat in the rent?	Yes No	Heat is included as part of your rent.
